_____________________________ has my consent and approval to attend and participate in the children and youth activities and events associated and organized by Central Chapel, 2546 Back Creek Valley Road, Hedgesville, WV, from July 1, 2025 to June 30, 2026.
I/We understand that my/our child will be required to adhere to and obey certain rules and guidelines as set forth by the leaders of this/these activities. Our goal at Central is to provide a safe, secure, and friendly environment which is conducive to teaching and learning Biblical truths and Christian concepts. Disorderly conduct will not be permitted and will incur disciplinary measures to maintain that environment.
Disorderly conduct is defined, but not limited to, the following definition. A student will not harass another student or another person or misbehave in a manner that causes disruption or obstruction to the educational process. Disruption caused by talking, making noises, throwing objects, or otherwise distracting another person constitutes disorderly conduct. Behavior is considered disorderly if a teacher is prevented from starting an activity or lesson or has to stop instruction to address the behavior. If my/our child fails to abide by the rules, I/we understand that I/we will be contacted and must come and pick up the child from the activity.
